Patients are seen by appointment only. To schedule an appointment, please call (303) 832-2344 during regular business hours. If you would like to be seen at a clinic other than our main Denver location, please specify where you would like to schedule your visit.
New patients are asked to arrive 30 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.
Our established patients are also asked to arrive 30 minutes early for our nurse portion of the visit and to collect any lab work needed prior to seeing your physician.
We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.
We will bill directly to most insurance companies, and this is done as a courtesy for you. However, it is your responsibility to understand your benefits, and eligibility. You are ultimately responsible for any payments if the visit is not covered or authorized. If you are unsure if your visit will be covered, we recommend contacting your insurance carrier directly using the customer service phone number on the back of your card.
It is your responsibility to understand the requirements of your insurance policy. If a referral is needed prior to seeing our physicians, you will need to obtain one through your primary physician’s office. If you choose to be seen without a valid referral in place, you will be responsible for any charges not covered by your insurance company.
Cancellations or Rescheduling
We understand that schedules can change on a moment’s notice; we do ask that you keep us informed as soon as you are aware if you are unable to keep your scheduled appointment and/or check-in time. In order to maintain our schedule, we do appreciate at least 24 hour notice for cancellations or rescheduling of appointments.
All prescription refill requests should be directed towards your regular pharmacy. If you are out of refills, they will contact us to review and authorize, so this is still the best way to refill your prescription. All requests will be processed in 24-48 hours. After-Hours & Emergencies
If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call (303) 832-2344 and our answering service will assist you.
